Children taken away/dying for no reason

Randomly got pop-ups that my child and teen were feeling neglected/their grades were bad, when all of their needs are met and they have A and B grades, respectively. About an hour of gameplay later, I got a pop-up that the child was being taken away due to neglect. Again, all of her needs were met and she had an A in school. Once she had been taken away, I got the Life & Death pop-up asking if I wanted to turn her into a ghost, and I chose to make her a playable ghost, but she just disappears. Her parents only have the "Child Whisked Away" moodlet, nothing about grieving a death. I even disabled my mods to see if I could replicate the bug, and always at the same time I get a pop-up stating that the kids are feeling neglected.
