Crashing PS5

Game crashes consistently at multiple points during gameplay. This is happening every single time without fail, making the game completely unplayable. I have done the whole clear cache on my PS5 and still the same results.  I have already cleared the PS5 cache and rebuilt the PS5 database in an attempt to resolve the issue Neither fix resolved the crashing.I own all major packs.

Expected behavior: Game should load and run stable during world travel and active gameplay sessions.

Actual behavior: Game crashes consistently at multiple points, every single time, with no successful session completed.

This issue is severely impacting my ability to play content I have paid for. I am requesting an urgent fix or patch for PS5 players. Many other console players are reporting the same issue following the April 16 update. I've added an attachment and that's all from SIMS!
