Yeah, I just noticed this as well. I had the baby quite a while ago as they're currently a toddler, but I just now looked at my sim's milestones. Toddler is not a descendent of grim, (doesn't have the trait or the birthmark,) nor does my sim know the grim reaper. So, I just removed the milestone with UI Cheat Extension. Seems doubtful that the milestone would have any impact on gameplay but who knows.
