[LD] Sim in another household not dying/repeat inventory message

I'm also having this problem, but I don't own LD

update: turned off all mods, and it only took 2 deaths until he actually died 

I have multiple played households, and I got notified that a sim died, except the notification kept coming multiple times after I have already selected what to do with their inventory.

Furthermore, the sim was not a ghost in the relationship panel (they are related to my current sims).

Decided to go to the house directly to see what was happening, and the sim was still there, then they began to die.

I collected the headstone, but then they respawned and died again and then again. 

Please see video attached. I ended up with 3 gravestones, and none displayed his name attached to them.

Also, for some reason their seems to be issues with the crying animation as seen below. happened to the son in the video and then later when the wife got back home. this was taken after the first death, but you can see the sim dying once again the bottom of the screen,

I do have mods on.
