I looked into this:
Every unsuccessful romantic interaction increases commodity_Death_BrokenHeart which causes the death by broken heart when the value exceeds 95. However, there's no way to decrease that value, and it takes about 11 sim days for it to reset to zero. Successful romantic interactions make the broken heart moodlets disappear faster but the actual commodity remains unchanged. That means even without a broken heart moodlet, one unsuccessful romantic interaction may be enough to kill your sim without a warning.
A solution would be to reset the commodity when the moodlet expires, so that there wouldn't be surprise deaths. The same way that other death types, like elder exhaustion, work.
Edit: there's another thing I noticed. Every interaction that the game considers cheating increases the chance of dying from a broken heart. It doesn't take into account any romantic boundary or trait stuff, at least kissing and woohoo are considered cheating if the sim is in a committed relationship with someone else, even if everyone is okay with it. Which means that polyamory is deadly lol. It makes no sense that the cheater is the one getting broken heart. It's found in file 129283<!--Buff_Jealousy_Cheater-->
