The game has situational outfits and yes, they can be really hideous
There's a few bug reports that might apply to what you are seeing, bit hard to tell from your post
- NPC Sims wear random cold/hot weather outfits not assigned
- Sims don't change out of situational outfit
- Generated NPCs wearing hats /socks/accessories on all clothes
Maybe it's all of them ?

That post is 6 years old but doesn't look like there has been any fix or answer yet. But yep, that first one is definitely the same issue I am having, just read through all the comments as well. And it has been getting worse over time! At least for me it has. It gets worse the more packs I have, I think. Because stuff becomes more random...
