A few minutes into playing after updating, my sim gets a call from her cousin to say that the cousin is now in a relationship with her own brother. I went into the other family to check and sure enough they have a romance bar, and are tagged siblings, sweethearts, and partners. This is NOT okay.

there's a bug where partnered Sims turn into siblings. Could you check on the profile if that's the case for you ?
I’ve been having the same problem, however this happened after both sims spouses aged up into adults.
This is happening to me too! It happened in both of my most recent play throughs of Cottage Living. I have no mods and haven't for a while. It happened in the first play through, so I tried a second play through and it literally just happened again. I turned the game off for like, 3 hours and came back and my wives were sisters! I hate this so much
I have a completely up to date game and do not play with mods, and there seems to be a bug happening where my happily married sims, (generally of the same sex) end up with a sibling relationship tag (sisters this last time) despite never being related, growing up together or anything that would confuse the game. They were created in CAS as partners, have had 3 biological children together, and were happily living out their lesbian country caretaker aspirations. Suddenly, they can no longer romantically interact. I started a whole new save, and the same exact thing happened again, literally 24 in real life hours later. What is the deal?
My engaged gay couple suddenly became brothers. The one who isn't supposed to be part of my family tree (Harry) suddenly is, their adopted daughter now has a "niece" relationship to the other one of the dads (Eddie), who was originally in my large family tree, they cannot have romantic relations. I felt like this happened after Harry also adopted their daughter - she was first adopted by Eddie via phone, but something else could have caused this. The relationships are not editable in CAS, the guys still show up as engaged in there with steamy dynamic and even though the option of "household member" is not greyed out, I cannot choose it, it reverts back to engaged. I cannot change the relationship with their daughter either, even with fulleditmode on, it says I cannot edit pre-existing sims.
My sims have been married for a while but just recently we noticed they haven’t had any romantic social options. I checked the relationship and it says they are brothers. We never made them brothers and want them to go back to being just spouses :(
I've had this happen a few times with married couples all of a sudden becoming super siblings!

could you check your family tree if the Sims became siblings in the family tree ?
Yeah, they are now listed as each others sisters
