Sim Dying but took almost 1 irl hour to be reaped

My Sim recently died in his sleep </3 but I had to reset my game five times for it to actually process- I would get the pop up, the animation played, Grim showed up, and... nothing happened. The game continued running, but was stuck in the "sim is dying on lot" so I could not build or save. 

I eventually got him to get reaped by completely closing the game and restarting (3 tries)- making sure he didn't die in his bed- but when he was finally reaped (which again took hours in game with Grim just... standing there) I didn't receive an urn or a gravestone until planning a funeral. 

I have MCCommandCenter but the issue still persisted when disabled.
