Not really a game breaking thing, but it really bothers me when I play the same family for generations and they never have any uniqueness to their features. Teeth are always perfect and they never get freckles or moles. They also never get birth marks. The only way one of my sims get this is through cheats to edit their appearance. I know that the skin details are supposed to be random...not sure about the teeth, but I know that there are other options then just the default.
