Parent is asked to perform an action, action queues, sits there, is never performed, action drops from queue.

This morning, I started a brand new family.. the 'Tester' family, with Jacob, Helena and Ashton Tester as triplet toddlers. They are in a different house to the family I was playing that was having the issue, and aside from the issue with reading to the toddler (a known issue), I am unable to recreate the glitch.
The game that is having the issue was started in late 2024, and I have just completed the Legacy Leader achievement (before now going back to playing just one family throughout their whole lifespans), so there is a lot of data. I might make some changes to it, eg marking some of the families as NPC, cleaning up files, and see if the issue still exists. Stay tuned.
I cleaned up my Sims 4 folder as per instructions crinrict has published elsewhere, and the problem still persists in that game. So, somehow the problem exists in an old save, but not a new game.
It seems to be confined to Care actions, both initiated by me clicking on the care actions in the menu for the adult, or the Help actions for the toddler. Spontaneous care still occurs.. eg, if the toddler wants a bath and asks the adult for one, the conversation will happen and the toddler will be bathed. But if I click on the Care menu and select the Bath option, the action will enter the queue, get stuck at the 'Talk' stage, and then drop from the queue without the task being completed.
I switched to playing a different family in the same save, and the issue was not there.
Went back to playing the family I want to continue to 100 generations, and it now seems fine. Let's see if it continues this way.
