I experienced this before though recently not.

Play on PC via EA app, no MOD/CC, shots taken in Apr,24,2023 and Aug,11,2023.

If you haven't seen it since 2023, it's unclear if it's still in the game and even more so it could be a mod issue for the OP. Something that got fixed somewhere along the link but not for a certain mod.

Do you remember if you created the Sim in CAS at the time ?

OK, I understand you said, that's right. And I get the feeling which update I don't remember, this issue fixed. Both of those infants are in-game born babies. The pics are took when change their cloth in CAS after aging up.
