I think you just forgot that you holstered your weapon before you started to heal. I use to think it was a bug back in the day but then I recorded my games and compared it with a key logger and came to realize this was the case. You holster your gun all the time, almost as a tick without thinking about it. Then you start using a heal. When its done, you return to the state (weapon up or down) as you were when you started the heal.
Also, canceling the heal takes ONE press of either the attack key, cycle/select weapon key or the sprint key, which also will return you to the state you were in before the heal was started. So if you start to heal with your weapon holstered and someone jumps you and you press "cycle/select weapon 1 or 2" you wont bring up the gun but just cancel the heal. SO if you press the attack key right after, you will start one melee attack. If you want to cancel the heal, bring up a gun and then start firing, you need to press, "select weapon" x2 and start firing. Ive done this mistake a thousand times =).
