Yes, yes and yes. I played Mass Effect 3 the original game multiple times and miranda always survived. I didnt do anything different this time. The reputation bar was full.

it could very well be that in both cases flags are set wrong.

Did you saw other posts about this two issues?

I did not find any more topics about Zaeed or Miranda dying in ME3 despite being loyal.

What platform are you on?

I'm using the Xbox series S. Two other things I notice in ME3, Kelly Chambers and the comic. I saved Kelly during the suicide mission along with the rest of the crew, Dr. Chakwas is even in the Normandy. No sign of Kelly Chambers anywhere during ME3. Also the Genesis comic didn't show up at the start of ME3 which is weird cause  usually there was an option to view it before the start opening cutscene.

@DanieruSempai  This doesn't seem relevant to the original post, but Kelly only shows up in ME3 if you have dinner with her in ME2 (in addition to surviving, obviously). I'm on Xbox One and she has showed up as normal. 

Oh yeah sorry, I could have sworn you could have met her in ME3 even if you don't romance her. Oh well my mistake.

@Psychotps  No, that's definitely always how it was for me. She never showed up in ME3 unless our relationship was strong enough that she would feed my fish.

@Fred_vdpStrange. I've played it more times than I can count and I've NEVER had her not show up. I've romanced her a few times but mostly I don't. She's still always showed up. Have no idea why yours didn't.

Anyway, I'm coming up on Miranda's mission. Just have to do Thessia first. I've actually never had her die in a normal play without a broken mod. If she dies in mine, that's a new bug. However I might be able to fix it with the save editor. I think I know the problem. Not long ago, the Project Variety mod had a bug that caused that exact problem. The fix it setting a certain bool value in the save editor. I'll see this afternoon if there's a new Miranda bug or not.

The Wiki says that having dinner with her is a prerequisite. Maybe your game was bugged. 😛

Well, there's no "Miranda Dies" bug.

I just finished Horizon and my Miranda didn't die. If others had her die, then they just missed something.

I did notice before I met with Miranda on the Presidium, I never got the email from her asking to meet there... I met with her fine though because I already knew I was supposed to meet her. If the newbs didn't know about it and never got the email... she'd die without you ever knowing why.

There obviously is a bug, because at list two guys here wrote that they did exactly same things they did in original ME3 and they still got Miranda dead nevertheless...

Did anyone find out about possible fixes to the issue? Also caught it ☹️

@KowaseruKurueru Did you meet Miranda in the apartment on the Citadel where she talks about the control chip?

I don't recall receiving the in-game mail to meet her there, so I'm curious if the bug is that players aren't informed about the final step of the side quest. I did meet her there, because she was indicated on the map, and she survived my playthrough.

There are only 3 reports about this issue and the game is out for over 6 weeks.

So while it is possible that there is a glitch in your game, there is almost certainly no bug.

Here are the things you must do or avoid to do if you want her to survive.

In addition, you must also meet her all three times it is possible on the Citable.

There are two Genesis comics, Genesis for ME2 and Genesis 2 for ME3.

@Fred_vdp I've met Miranda in the apartment and we had conversation about the chip. I just checked in-game - I also did not receive email from here to meet there, I just noticed her on map as you did and went there.

@holger1405 I did everything due to these statements, except that I missed her first meeting on the Citadel.
But I did just the same steps during my both playthroughs of original ME3 several years ago, and she survived.

I googled some old themes from original ME3, and several guys also had this issue. Some of them stated that certain "checks" in their save files were incorrect, and they managed to fix this by changing some debug lines in save editor.

Afaik the first meeting is a must have event. 
If you missed it you will not be able to save her. 

I don’t know if the MELE save editor has the necessary flags to change to make it work and I am mobile itm so I can’t check. But you could of course try. 

Then how come @Lasadon and @DanieruSempai say that they've met Miranda all 3 times and also have her dying?
