I dont think anyone knows exactly how the penalty timer build-up works but it builds up exponentially and seem to reset VERY slowly. So you can basically play 50 game without leaving and leave once and get 5 minutes, then play 50 more games without leaving and when you leave the second time, you are immediately hit with 15 minutes etc etc.
And to answer the second part: If they gonna remove the abandon penalty for 3-man premade squads, they also must remove all kinds of loss forgiveness for 3-man premade squads. Otherwise premades could abuse this to gain rank, i.e. having one player faking a DC every time the team is about to lose RP to give the other two loss forgiveness. Me personally would be totally OK with this trade-off since either me or the premades that I play with pretty much NEVER have our game crashed but based on the numerous threads about game crashes on this forum, I would say that many people would start complaining if they implemented this since they blame it ALL on Respawn, unlike you who actually recognize that the reason for game crashes can also client sided issues, like unstable connections and computer builds.
But I guess one simple solution to this would be a "leave forgiveness"-button in game, for instance in the squad tab, which allows who two players in a squad to "forgive" the third player for leaving. So if both two players click this button before the third player leaves, he wont get punished for it.
Yea, I guess there should be a choice between loss forgiveness or no abandon penalty for full premade squad. :D
