I fixed the issue! In Radeon Settings, Global Wattman, I reduced the Power Limit from 50% to 40% and that did the trick.

Be careful with that, considering you have your card OC'd,

by lowering the power limit you may cause your OC to become

unstable.

I would suggest lowering your OC along with voltage.
