Are you swapping the gun before the reload animation is fully completed? That's what happens to me when I pull out my other weapon again with an empty mag.

No mag. Only one gun, reload the gun , go to start shooting it double reloads. Another time picked up a gun fresh off the ground go to use it and it reloads
