I was thinking this may be the case.
But in the clip i posted before i don't pick up a single attachment after the complete reload animation of the spitfire.
If i did can you give me a time in the video where i pick it up.
Im only human maybe i keep missing it ^^.
Also i have this problem with all the weps.
But some weps just have a realy fast reload animation so its not a big issue there.

Before i go into combat now i always triple check my weps now ^^
Its getting a lot better with the checks but sometimes i still die to this and its realy frustrating.
OK so, I've watched the clip again and here is my (untested) theory.
You had a Hemlok, switched to the Spitfire, immediately all the attachments from your Hemlok went to your Spitfire (you did that initial 'cocking of Spitfire' animation but that was as you dropped your Hemlok & picked up the Spitfire - at no point did you ever equip your R99 and go to the Spitfire from your R99 {in other words, you never went from 1 -> 2, only from 2 -> switching weapon on 2, and then from 3 -> 1, then eventually from 1 -> 2 and that's when it did it).
For the rest of the clip you only went from your, now, Spitfire, to 'no weapon' and switched back a couple of times to your R99.
Maybe you needed to go from your R99 -> Spitfire at least once, before the engagement? I am guessing, because of the attachments that were added when doing the Hemlok -> Spitfire Transfer. I am pretty sure that would've canceled out the animation?
I'm glad I was able to help a little; but you still say you have some issues with this so here's what I suggest (until they fix, at least), because ever since I did this, I have not had one-single-instance of this happening to me (and before I was like you, dying all the bloody time to this 'bug').
When I'm not looting / When I'm moving to a new location / When I am catching up to my team-mates before an engagement, I always do this pattern:
Weapon 1 = Key 1
Weapon 2 = Key 2
No Weapon = Key 3
I do 2 main moves, the first became second nature, after 250 hours of playing APEX, and the 2nd is for 'good measure'.
First move: 3 -> 1 -> 2 -> 1 -> 3
Second move, I do separately: 3 -> 2 -> 3.
I will also occasionally spam: 3 -> 1 -> 3 // or // 3 -> 2 -> 3.
Don't do it so fast that the full animation doesn't load, of you even holding the weapon.
If you still get issues after that, then it's probably time it was properly tested (but honestly, I'd much rather just play APEX rather than sit there & test stuff).
