Not entirely true. It's 50 million (give or take) individual accounts that have downloaded and played the game at least one time. If it were literally just accounts, it would make no sense, since you need an Origin account and there are well over 50 million of those since Origin has been around since like 2010 or 2011. There is no "Apex account" so you can't look at this number as "accounts created" which is how Blizzard markets their Overwatch numbers, it makes sense if Blizzard does that because an account costs money to open, so their number of accounts are directly tied to sales of the game.
The only people making duplicate accounts would be cheaters trying to bypass their bans. There is currently no reason to open multiple Origin accounts just to play Apex on more than one account...this game has no ranked system and there isn't any way to smurf, everyone is in the same boat, you might be paired with a level 1 noob or a level 100 guy with 3k kills. I'm not saying this makes it impossible for people to open a bunch of Origin accounts and download the game, I'm just saying I find that UNLIKELY because there is no incentive. It's very different than when people open new accounts in League or Overwatch.
Also keep in mind that Respawn is aware of this, that's why they advertise a "total" number and a "concurrent" number, the latter tells you how many individual players have been online simultaneously at peak.
