My account was hacked 4 years ago, but I left it alone. I recently had it restored, but it was banned, so I'd like it restored.

Unfortunately we can't help with bans here on the forums. You can always make another appeal with our terms of service team, and they'll take another look at this for you.
I contacted EA Help but was told to decline.

If they decide to uphold the ban after around 3 disputes, then the action is likely final and nothing can really be done to change the fact.
