yes.  it gives me an error 404. 

I just followed the links and got as far as the form. Did you get to the form and then get a 404 on submission?

yes, I filled both forms. and after filling out each, I confirm - and it gives me an error.

This might have been a temporary issue, checking now I was able to create a test report. 

Looking again now it seems you've created a report recently, has this become unstuck for you?
