I am also experiencing this issue. I have made a report on it too. It's weird as none of my friends have experienced it, and I am unsure if random squad mates have the issue too. I didn't notice until reading comments and thinking about it, but it does only seem to happen in close quarters combat. It's weird and I hope it gets fixed soon.
