Code cloud on Xbox one

For many seasons now, my game will freeze then a moment later it will disconnect and when you try to reconnect to the game it doesn’t work and you just get kicked. This is frustrating especially in ranked as then you get a leaving penalty for something that isn’t your fault. Can you please sort this out. 

My router directly connect to my Xbox and I’ve tried restarting everything, I uninstalled and nothing works. This does not only happen to me but many other people I play with as well
