Give this one a try: https://answers.ea.com/t5/Technical-Issues/Possible-fixes-to-2-crash-issues-DXGI-amp-NO-ERROR-PC-Only/m-p/8730966#M62856
This solved both issues for me including Access Violation 0xC0000005 (NO ERROR) problem.

@just-brajan umm.. the DXGI Error is mostly because of an unstable oc, not because of drivers.. When i OC my gpu too far, even tho there are no artifacting etc., i get the same error. When i go back to my all day settings, the DXGI Error is gone.
- 6 years ago@Xenacftw Not exactly. OC increases the chances but i was getting it even on clean OS with no OC whatsoever.
With registry tweak i can run Apex with or without OC with no DXGI errors but of course it might be different between systems. - 6 years ago@just-brajan well, even a factory overclock can be too much for Apex / a GPU. Even Nvidia's "self OC" for the GPUs can cause issues. I will try your reg. tweak later, maybe i can push my GPU to 2.3ghz without crashing.. Then you have my lifetime respect :D
