If you're on Windows 8 please make sure you have the latest updates installed for your OS.
Download windows 8 updates:

We are looking at a lot of DxDiag reports and we're seeing a significant trend of users not having the latest Windows updates installed and outdated video card drivers. You'll want to go to Nvidia and AMD's website to make sure you have the latest one.

Windows updates can be found at Microsoft's support site linked above. You can search "Windows Updates" in the search bar on your desktop taskbar to see if you have any pending installations.
