Bring back formation subs

@rocthabeast Its needed than AI telling me whats best to put on the field.

@rocthabeast While this can be done in-game (using triangle/Y when on formation selection), we need it in the main menu in Dynasty!

Current implementation the subs have to be done every game, and you only have a few seconds to make the subs in-between plays.
