My team is not fielding 11 players on offense or defense

After year 2 of dynasty mode on Xbox, my team doesn’t field 11 players in all formations on offense or defense. WR’s, RB’s, or LB’s are missing or a Punter will be positioned where the RB should be. ...

For anyone else having this problem, are you using formation subs?  It seems to happen most with certain formations, and I'm wondering whether it's related to the fact that I've formation subbed in those formations – IOW, what does the game do when a player that is formation subbed into a spot becomes unavailable?
