Community AdminUpdate: Thanks, I got a save file and this is definitely replicable now.
However when I ran the Op normally on my personal playthrough it went without a hitch. Can I check if folks who have this issue are still at Haven, or have you moved on to Skyhold?
Do you have Investigate the Chalk Markings completed/initialised?
Sounds similar to my issue below. Wish more people had my issue but I am going to guess majority of people past that part of the game much sooner then I did, so I am here stuck at the start because I bought the game later thus by myself with this issue ☹️

It seems to at least be in the same ballpark. APfB had some issues pre-patch, so I suspect it's related to the fix, but more info always helps. I've asked for some more over on your thread.
