Finally get the game working.. been playing for 2 days, and its so damn bugged that i can't even see half the people.. I have not been able to talk to cullen once, and now cassandra has dissapeared.. Is there an expected patch coming out to fix this issue? its happening with ordinary npcs as well..
I'm speaking for PC versions.. My friends on console have not had 1 issue yet...

This is from Bioware, so hopefully the nvidia issue gets fixed soon:
With the release of Dragon Age: Inquisition, we've been hearing a number of concerns from members of our PC community, including:
- Driver support
- Hitching and frame rate issues in cutscenes
- Feedback on keyboard and mouse controls
- General user interface optimizations for PC
We've been following these issues closely and are looking into them.
Player experience is a top priority for us. Our goal is to address as many of these as possible in our upcoming patches, including some in our next patch, which is underway. We’ll release further details in the coming days and will continue to monitor your concerns.
Your feedback is important to us, and we appreciate your contribution to making Dragon Age: Inquisition a better overall experience.
UPDATE:
If you are having DirectX issues and you have an AMD Radeon graphics card, please update to the AMD Catalyst 14.11.2 beta driver.
We are aware of issues with certain NVidia graphic cards and we are working with NVIDIA for a solution.
Hey guys.. solved my problems by clicking the game inside of origins with the right button, and choosing something like "verify updates" (my version is portuguese so i am not sure what the exact wording is in english) .. it took a few minutes to do that.. and after that worked perfectly .. good luck for you guys..

This, sadly, did not fix it for me and many others as well. I was able to play the game until an hour ago. The game started to behave oddly and started to drop the fps... I then closed the game and suddenly an Origin update popped up, restarted the app and now I am not able to launch the game. I am not able to play neither ME 3, Sims 4 nor DA Inq. I am able to play older titles like ME1 and DA Origins.
Do you know how to fix this?.

Exactly the same thing has happened to me, i installed the origin update this morning and i've not been able to lauch the game since.
Update:
Installed AMD Catalyst 14.11.2 beta driver and ran 'Optimise settings', seems to have done the trick.
But I have a NVIDIA 680M card, the driver is updated and the game optimized. So I don't know what else to do. The game was running perfectly before the Origin update I just mentioned before.
