Well, I just offered the following advice to someone with a new system build with Windows 7, but much of this also applies to Windows 8 and 8.1. Try any or all of the pieces listed and see if it helps any.
Some things to try:
Go into your game install directory on your hard drive (i.e. C:\Program Files (x86)\Origin Games\Dragon Age Inquisition) and dig down into the \__Installer\vc subdirectory. There should be 4 folders inside, each with a Windows executable file inside it. Run each of them and install them (select REPAIR if they ask you to repair or remove).
Afterwards go grab the DirectX WEB installer from here: http://www.microsoft.com/en-us/download/details.aspx?id=35 save it and run it. It will look at your installed version of DirectX and repair or update anything from DX11 plus install the files necessary if you also happen to need DX9 for other applications - Fresh installs of Windows do not always load all of the files necessary for the older versions since the most recent Microsoft updates to Windows.
(Additional note here - Windows 8 and 8.1 no longer even include the base framework for DX9 and below. I have found that even some applications that swear the rely purely on DX11 won't run properly unless DX9 files are present)
Then just to make sure Windows has all the updated bits, you may also want to download and install this: http://www.microsoft.com/en-us/download/details.aspx?id=30653 it's .NET Framework 4.5, as if you are running the latest drivers for your video card, they require this to work properly but the installer may not install it depending on your Windows security settings. (There are also a couple of higher versions you can dig up for Windows 8/8.1 if you want to be sure you have the latest and greatest. You should be able to find them listed under Optional in Windows Update.)
After that, be sure to check Windows Update and install anything lying in wait for you there.
Finally for the Windows side of things, I strongly recommend reinstalling the latest video driver from NVIDIA (347.52) http://www.geforce.com/drivers/results/81877 using a "Custom" install, checking "Do a Clean Install" and NOT installing GEForce Experience.
(Sticking with the other driver I posted about is also an acceptable compromise since NVIDIA released it especially for issues related to this, but if you have updated Windows, I do recommend doing a reinstall just to be safe.)
After all the Windows tweaks, be sure to reboot at least once.

Vsync Adaptive - this one is crucial, any other setting and it's crash city.
In Game Post Processing Effects LOW - again critical, crash city if not.
Lastly - try right-clicking on your game tile for Inquisition in your My Games tab in Origin and selecting "Repair Game" and let it check your files. They just pushed a patch, and it is possible your files are not completely up to date.
