Chest completion not working?

I have been looking over & counting how many chest I have found recently trying to discover everything. So far so good but I came across the Necropolis, it says 16/18 chest collected with 88 discovered. I have counted countless times from the map HUD & I have all 18! What is going on? Could be happening to different locations as well in the game like treviso. Since there is a location with a chest I have been waiting to get to since the start of the game & I am almost completed the game & have yet to get that chest. Honestly Great game but so buggy & confusing. Please help devs. I’ve been playing dragon age for years. Anyone else come across this as well?  

Update 11/15 : I am still having the same issues with the necropolis chests but now am having issues with treviso (chest is at 14/15 with 93% discovered) and arlathan (chests at 12/13 and stuck at 95% discovered) as well I am stuck and cannot get to a 100% like I planned from the beginning, I am very very disappointed and upset. Devs you gotta put out an update before I finish my playthrough PLEASE!! 

Update 11/16: I currently found out that the shadowy grove puzzle was incomplete and had just finished that and got arlathan to 100% with all chest found. But treviso is stuck still at 93% with 14/15 chest and I have counted all and I do have all of them I have even seen others playthrough and they have discovered all places that I have on the map including all same chest. Same with the necropolis it’s still stuck at 88% with 16/18 chest. Devs I am about to finish the game, I need this atleast fixed before my second play through, I was hoping to beat this game 1st 100% completion but you guys have let me down on that front. Disappointed to say the least.
