Few bugs - character appearance + codex entries

- Inky's face suddenly changed during the final scene with Solas. It looked like it's changed back partially to the default look, even though she was ok during previous scenes.
- During romance scene with Lucanis, Rook's face expression was umm weird at least, she was smiling like a psycho with a crazy cross-eyed look, which completely destroyed the scene lol
- Another thing that I've noticed is that Harding appeared dead in the codex, while being alive. I sent Davrin on a suicidal mission and he died, but there was no codex entry about it, only entries about people crying over Harding, who was alive + Bellara was talking about Harding being dead.

I tried to reload the scenes few times, but it didn't help. I didn't have those bugs on my previous playthrough. I'm not using any mods and I'm playing on PC.
