game changes character appearance mid game

so this has happened a lot of times, I'll be playing the game and a cut scene happens and then my character is no longer how I made him yet it still has the male voice. not sure why it happens, but the only thing that seems to fix it is if I exit out of the whole game which is annoying af. the picture is what my rook changes into and not what I had created.
