So when I reloaded back into veilguard my rook head presets went back to default but as soon as I went to the mirror of transformation everything appears to be fine I tried reloading some other saves but it seems to persistent.

I’m having the same issue.. some saves she’s fine, others she’s not. though in the mirror my rook is always still the default not my custom...
yeah i had to go back to a way earlier save like almost beginning of game to get my original rook back so like at least I wasn't that far in the game at least, still sucks tho
