Inconsistent codex entry near end of game

In the midst of The Last Gambit quest, I noticed a new codex entry in The Elven People section.  It was from one companion about another companion, titled Lost Friendship.  I don't want to go into details of it due to spoilers, but the companion being spoken of is inconsistent with the choices I made and outcome of the Isle of the Gods quest.
