Refunding skill points reverts back to default ultimate

I'm having a similar issue where I feel like I come back into a save and my ult has reset to the basic one instead of the one it was on when I saved and exited, I'm now also noticing whenever I use new skill points even on companions it resets the ultimate I had selected on my skill bar.
