Help with a ban appeal

I've tried making multiple attempts on multiple devices for a ban appeal because the reasoning was non specific. I just don't understand what i did. I've attached screenshots for reference. I tried the alternate submission route but it just sent me to articles. Please help

    Nobody can provide you with any information regarding bans on the forums. Bans are handled by the EA ToS team.
