I disconnected all external USB devices and tested again. The issue still occurs. I have also tested a different controller and the problem remains. Because the issue happens with multiple controllers, it does not appear to be a controller hardware problem. What additional troubleshooting steps can I try?
Everything is disconnected except for the controller.
