I just purchased the premium seasonal track pass and claimed rewards, on the selection page after, the 93 Rated FUT Birthday Rivaldo did not feature, nor does he appear in my squad. I have reset UT but haven't seen him, unsure on how to figure out how to get him / reset.

Try to search your club for an item rated 93 to see if you find him.
I'm asking that because sometimes some items you claim are sent directly into your club and don't appear correctly in the filter.
You can also check the companion app to see if you discarded the item by accident.
