Well just despite Nvidia and EA going dark on this I have found a legit workaround that allows you to play with an unlocked framerate on 5000 series GPUs.
You have to create a game profile for Madden 26 in NVCP and enable "Prefer Maximum Performance" under power management mode. This workaround resolves the crash, however, EA and Nvidia should not stop trying to actually fix the issue as its related to P-states. Basically the GPU has wild power fluctuations resulting in the crash. Prefer maximum performance always keeps clocks speeds at their maximum ensuring stability.
This should not be required, fix it.

This does not work. For me the only thing that works consistently is to lock FPS to 60

It did for a few days, then the most recent Madden update actually broke that workaround

New Build offers more detailed error message. Its now pointing at C++ source file: swapchainpcdx12.cpp.
I received error on first open of the new update from 9/17. I left the system idle at this screen while I made coffee. Came back with my fresh brew to see this error.

This is somehow still unaddressed after two months time... the latest Nvidia driver today does nothing to fix this issue.
