Ancient Debris on Trebin kills all controls

@EA_DavidI believe I also had this issue (it was ancient debris, can't remember what planet). I *also* had this issue near a mineral node on a different planet. I was unable to save/load around the issue for the mineral node because it was a "Hazard" planet, so I was taking damage and not allowed to save or return to Normandy. Play primarily with KB/Mouse, but I do have an XB360 controller (wired) plugged in.
