Help!- Completed Banner (Cerberus Mastery) will NOT equip and challenges stuck at %100 on PS3 (ME3 Multiplayer)

I have completed Cerberus Mastery on PS3 ME3 multiplayer- the banner is visible on the challenge screen, but I cannot equip it.  When highlighted, the banner says 20 out of 21 challenges are complete- yet ALL of them are listed at %100 completion.  Cerberus Mastery is listed as incomplete under the Mass Effect Challenge as well.    

In addition to not receiving further challenge points for Cerberus (I have almost completed Geth and Reaper twice- both were done around the same time as when I completed Cerberus), I am concerned about not being able to get the Mass Effect banner once I have completed the other challenges.

Has anyone else heard of this and does anyone know how to fix it?  Thanks in advance for any info you can relate😕mileyhappy:
