Paramore III achievement still bugged... Quite frustrating since it's the only one I'm missing and I can't make it to work.
EDIT : Apparently after the fix, you can have paramoure II achievement because of the fix but the game still doesn't track you had a ME2 romance prior to the fix, and paramoure III doesn't works. It may also be because you romanced someone who doesn't count in ME2 (aka kelly, samara etc...).
In this case you need to replay a ME2 romance and paramoure III will unlock.

Insanity 2 didn’t pop for me after beating the suicide mission. It was set to insanity from the beginning and was never changed. I even tried reloading and replaying through the mission 2 more times and nothing. I’m on PS4 so it’s not solely tied to Xbox. This is after the latest patch.
If only they could communicate about actual fixes as vigorously as they save us from big bad third party links.
In my original playthrough, I had a save right before the suicide mission and did the romance several times and the achievement never popped.
I went back after the patch released and tried again and still nothing.
The only thing I could think of was that my first run through of ME2 was before the patch and for some reason, the patch never applied to that game save. It doesn't seem plausible, but I'm out of better ideas.
I'm currently doing an insanity run of ME2 so hopefully, it works this time.
I found a workaround at least for me on PS for insanity 2. Not sure if it would work for others but you can always give it a shot. I completed both ME and ME2 on insanity and didn’t get 2 after beating the game. Reloading the save prior to the fight with Saren also didn’t work for me. I had to take some additional steps from just beating him.
There was an old workaround on last gen that I attempted and it worked.
1. load the fight with Saren (right before the second battle where he transforms)
2. Complete the game
3. reload the save and change the difficulty to insanity and make a new save
4. Start a new game and import the character you just beat the game with
5 change to insanity while on the Normandy
6. land on eden prime and make a new additional save
7. reload the fight with Saren you changed the difficulty on and beat him
this got insanity 2 to pop for me, maybe it will help someone. Maybe it’s an issue where it’s not recognizing the imported difficulties across the games. From other posts I’ve read, it seems to be glitchiest when you import rather than beating all 3 games on a separate character.
Made an account just to post this. If anyone else is struggling with this achievement, follow shepardessia's advice above. I beat ME2 twice on insanity. No achievement. Went back and beat Saren three times in ME1 on insanity. No achievement. Followed the 7 steps above and boom: achievement.

I'm about to start ME3 and am expecting to run into the same issue. Any idea if this workaround will work for Insanity III?
