Insanity/Paramour 2&3 still haven't unlocked after recent update.

In my original playthrough, I had a save right before the suicide mission and did the romance several times and the achievement never popped.

I went back after the patch released and tried again and still nothing.

The only thing I could think of was that my first run through of ME2 was before the patch and for some reason, the patch never applied to that game save. It doesn't seem plausible, but I'm out of better ideas.

I'm currently doing an insanity run of ME2 so hopefully, it works this time. 

I found a workaround at least for me on PS for insanity 2. Not sure if it would work for others but you can always give it a shot. I completed both ME and ME2 on insanity and didn’t get 2 after beating the game. Reloading the save prior to the fight with Saren also didn’t work for me. I had to take some additional steps from just beating him.

There was an old workaround on last gen that I attempted and it worked. 
1. load the fight with Saren (right before the second battle where he transforms)

2. Complete the game

3. reload the save and change the difficulty to insanity and make a new save

4. Start a new game and import the character you just beat the game with 

5 change to insanity while on the Normandy

6. land on eden prime and make a new additional save

7. reload the fight with Saren you changed the difficulty on and beat him

this got insanity 2 to pop for me, maybe it will help someone. Maybe it’s an issue where it’s not recognizing the imported difficulties across the games. From other posts I’ve read, it seems to be glitchiest when you import rather than beating all 3 games on a separate character. 

Made an account just to post this. If anyone else is struggling with this achievement, follow shepardessia's advice above. I beat ME2 twice on insanity. No achievement. Went back and beat Saren three times in ME1 on insanity. No achievement. Followed the 7 steps above and boom: achievement.

I'm about to start ME3 and am expecting to run into the same issue. Any idea if this workaround will work for Insanity III?

I don’t understand why I’m reading that achievements work flawlessly on PC and PS. I have beaten Mass Effect 3 on Insanity four times now on my PS5 and the Insanity III trophy still won’t pop up. I also still cannot trigger Paramour II or Paramour III. I have simply given up at this point.

I just beat all 3 games on insanity without changing the difficulty. ME1 and ME 2 popped without an issue. ME3 wont pop. Is there a fix coming for this, or is there a work around? Any help would be greatly appreciated!=]

Just had the same thing happen to me. ME1 and 2  insanity popped fine, but me 3 did not. i didnt complete everything mind, but enough to be over min war effort bar. I also had a mod installed which seaching online said didnt effect achievements, and didnt stop any others in the game and didnt effect ME2 with mods.  Wish we would get some form of confirmation it will be looked at. Hate to have all but 1 achievement unlocked.
