Mass Effect UNC: Espionage probe
Verge Cluster, Amazon system, Agebinium planet; went to Distress beacon in the mine, have to disable hard point on the bomb.
I do so with 2-3 seconds left on timer, get XP then the bomb blows up...

There are three hard points on this Bomb, did you disabled them all?
If so, try to repair the game. (in Origin > Game Library > right click ME2 > Repair) ((in steam > right click at ME2 > Properties > Local Files > Verify integrity of game cache.)
