My squad mates die even though they are all loyal

Could you share what combinations you tried, which characters were loyal, and which characters died?

All team member is loyal !

1. Specialist   -    Legion

    Fire Team   -   Garus     - Legion died

2. Spc.            -   Jacob

    Fire Team   -    Miranda  - Jacob died

On long walk always use Samara

If Garus leads second team - got a shot but survie !

For team escort send Mordin -if not he dies !

I play it the second time over,and save before Hit the Omega 4 relay, and now i try diferent combination . No one help. This hapening me the first time, i own the games Mass Effect 1,2,3 on Pc DVD ,and  on Origin (1,2 twice - buy once And buy 1,2,3 in pack) In Mass efect 2 buy every DLC but Kasumi loyal mission got a bug cannot play ! And i buy MassEffect LE - Mass effect 1 got the audio bug spend some monts for change audio (frekvency) Hz,but not help. Why not patch it up ??? -the original game dont have that bug,and still playable .

Are these two instances of the vent phase?

In the first scenario, Legion dies if he's not loyal. Did you succeed at the persuasion check when he argues with Tali? If not, you lose his loyalty.

In the second scenario, Jacob dies because he's not a suitable vent specialist.

@gyurkillSoo how i played the game ? I completed all the main and side quests, Only "Reaper IFF" remain- where you get "Legion"
All team mates loyal,and have all upgraded. Played The game second time,and try the Suiside mission run the forth time!!!
I think about the combination i dont have tried or what could be wrong...
Normaly i choose diferent team every battle ,if you now the mission You can bring that member what is needed for the game style, to protect me or make more damage ...
So back to mission , i did the Reaper IFF-got Legion after that
The collectors take the crew,only legion loyal mission remains.
Got the loyal mission , now i have all team member -every one loyal (with max paragon points,you dont have to choose between Legion/Tali,Miranda/Jack)
I hit omega 4 relay,talk... video...talk...video...

Now the first you need to defend the hangar :
Shepard - Legion/Thane

Suicide mission :
Specialist - Kasumi
Fire Team - Garrus
Shepard - Legion/Thane

Escort the crew :
Send Mordin because once already died on the long walk

Long walk :
Use Samara
Shepard - Legion/Thane

AND GOT "Now one left behind" MEDAL ! I got it !!!
