I haven't seen this before. Have you tried with different squad members? Did you modify the save in any way?*
* I ask because the only time I've had really odd things happen is when I've touched the save once too often.
(Example: Male Shepard in a romance with Traynor...😕mileysurprised🙂
It seems to be a really rare glitch or bug or whatever.
I did try with a different combo of squadmates, but that didn't help.
And I haven't modified the saves in any way.
However, I did get past it by exiting the game, preparing as I normally would (picking squadmates, allocating points, gun prep), and pressing the space bar while the mission was loading. That seemed to have triggered the scene properly.
In case anyone else runs into this weird glitch.
