Some achievements in mass effect 1 from the legendary edition are not unlocking pertaining the squad members where you have to complete 5 missions each with every squad member. Only missing two of those achievements and I've completed 5 missions with the last two members. The sentinal and the asari squad member.
Do it on the Citadel, it's the surefire way.
The game detects UNC assignments in a weird way... For example, the Helena Blake questline ending (where you just go & talk to her) counted towards my achievements. Countless other offworld missions did not. No bloody idea why.
I know there are some quests that only end when you report back to Hackett on the Normandy (hence there are no active squad members), but the ones that simply go into completed status while you're offshore also failed to trigger the achievements as well. I haven't been able to find a pattern in it...
It feels bugged as hell, but if you haven't completed the Citadel assignments, it is really easy to quickly grind almost each squad member achievement out during your first and second Citadel visit. Here's a list of every Citadel assignment in case you missed some.
Also, there's Garoth's privateer quest and Kahoku's quest (first phase), on both occasions you report back to them in the Citadel Tower - those also counted towards my achievements.
