for the custom content, couldn't we just put guidelines and stuff to prevent it? similar to what we already have for team name creation. although we could have full custom logos have to go through a "preview" from the devs before allowing it or something, but thats a lot more work

Community ManagerPeople will always find a way around the restrictions. Just like they do now with names.
As far as manually reviewing content goes, that could take up a huge amount of time when developers should be working on the game rather than keeping offensive content out. We'd need a separate team to do nothing but handle offensive content. I'm not saying that's impossible, but I don't see it happening. Regardless, we would love to see more custom content in game. Hopefully, we can find a good way to make that happen.
