Missing Items - and Broken Link to Submit Form

Hello - On release day, I tried to use the form that was provided on this site to request that some missing items be researched, and it must not have gone through because I have not received a response.  I went to use the same form to submit a "ticket" to ask for status, and I realized the issue -- it appears that the "submit" button is entire broken entirely or is broken in Chrome for MacOS.

So.....can someone take a look at the coding on the "button" but also reach out to me with the right contact information for how to ask for some missing items to be researched?
