Xbox - Player 2 Cob only fires once per round

In Co-Op Survival Endless, the Cob Cannon for Player 2 will activate, aim but only fire the first time in a round.  All subsequent attempts go thru the visual motions, but the Cob does not fire.  It works fine for Player 1.

This makes the game not possible to play and should be considered a show-stopper bug.

Also, as reported elsewhere, the progress for Co-Op Survival Endless is not being saved.
